The University of Washington’s self-insurance program insures events, retreats, conferences or other events sponsored by UW departments. The Office of Risk Management issues certificates providing evidence of coverage for events and vehicles.

Events & Activities

Please provide the name of the event, the dates and place where it will be held and the name of the sponsoring UW department. Please also provide the name, address (or e-mail or fax number) of the person who should receive the certificate.
