Individual Field Trip Application
Use the Individual application if you are a student who is purchasing the insurance for yourself.

After you fill out the form:

1. Print the confirmation email you receive
2. Take the email to the cashier’s office
3. Make payment at the cashier’s office

UW Department Field Trip Application
Use the UW Department application if you are a UW Department purchasing the insurance. Your budget will be billed by the Office of Risk Management.
