Forms

We require original forms for the following:

  • IRS Form W-4 (If not entered online using Employee Self-Service (ESS).  Foreign Nationals must fill out a paper form.)
  • IRS Form 8233
  • IRS Form W-8BEN
  • IRS Form W-4P (Cannot be entered online using Employee Self-Service (ESS).)
  • USCIS Form I-9
Below is a listing of forms available from the Payroll Office. Some of these forms, W-4 form and direct deposit, can be entered on line through Employee Self-Service
 
Electronic signatures are unacceptable, with the exception of verification of employment requests.

Foreign Nationals

Please use Glacier to populate and print all forms except for:
USCIS Form I-9          (Instructions F1, J1, H1B/TN, EAC)  
Note: A new Form I-9 is now available from USCIS, effective 11/14/16, with an expiration date of 8/31/19. The previous version may be used until January 21, 2017. Effective January 22, 2017 any old forms will be invalid and will be returned to the department. The new form can be completed electronically, however electronic signatures will not be accepted. The form submitted to the payroll office must have the original signatures.  If you have problems viewing the Form I-9 try using Internet Explorer.
