Form I9 (Employment Eligibility Verification Form)
The Department of Homeland Security requires that all new and returning employees complete a Form I9. Section One of the form is to be completed by the employee, all other sections are completed and submitted by the appropriate person in your department. It is required that the Form I9 is completed within 3 business days of your first work day. Please check with your supervisor to determine who will be completing the Employer section of your Form I9.
The Form W4 is for Federal Tax Withholding. Each employee is responsible for understanding their own tax liability. If an employee does not complete a Form W4, their taxes will be withheld at the highest tax rate. For more information about taxes, please visit our Payroll Tax webpage. The Form W4 can be filled out online in Employee Self-Service. Foreign Nationals are required to fill out a paper Form W4 with specific instructions.

Home and Campus Address
All employees are required to keep their home and campus address information current. Address updates should be done in Employee Self-Service.
